Root Cause Failure Analysis
A Two-Day Course for Line or Staff Personnel Involved With Finding the Real Causes of Problems
The course is presented over two full days and is a good blend of both lecturer presentations and case studies. Delegates are introduced to the concept of failures and failure management before they are introduced to root causes and root cause analysis. They learn the disciplined approach required for successful root cause problem solving.
Delegates who attend this course learn about failure types and the different levels of failure causes. They learn how to define a problem and how to collect and protect information. They further learn how to identify any conditions that may contribute to the performance of the task through the use of Task Analysis, and examine the task in depth through using Change Analysis. In addition, they are taught a number of tools that identify potential causes including Control Barrier Analysis and Event Causal Factor Charting. These analysis tools are intentionally limited to 5 or 6, covering the main ones only. Failing this, analysts tend to become confused with the various tools and invariably over analysis the problem.
Throughout the course the theory learnt is re-enforced through the use of exercises and a case study. Delegates are invited to analyse one of their own problems should they so desire.

Provided with the discipline and tools to undertake their own root cause analysis and arrive at the true causes of problems – if these are identifiable. They will also learn how to develop corrective actions and how to implement these. However, we wish to stress that we are only able to teach the process, tools and techniques. We cannot teach the logic and ingenuity required for successful problem analysis.
